Mae sure you have review the checklist for Certification Renewal and completed all of the steps before completing this application.

In order to complete this application, you will need the following information:
- Contact information for you and your supervisor.
- Documentation in PDF format of your prior certification.
- Documentation of how you satisfied the hours for renewal.
- Any information related to equivalencies already approved
- Make sure your priest has received the Priest Reference Form and is prepared to fill that out and email to certification@shms.edu
